脑梗死患者PAI-1基因启动子区域4G/5G多态性与血浆活性关系的研究

摘    要:目的探讨纤溶酶原激活物抑制剂-1(PAI-1)基因启动子区域的4G/5G多态性在脑梗死(CI)发病中的作用,并分析PAI-1基因4G/5G多态性与PAI-1活性的关系。方法随机收集130例CI患者,100例健康人作对照。等位基因特异性引物PCR分析PAI-1多态性基因型(4G/4G、4G/5G和5G/5G)。底物发色法测定血浆PAI-1活性,酶法测定血清三酰甘油(TG)、总胆固醇(TC)、血糖(Glu)。结果PAI-1基因型分布频率及4G和5G等位基因分布频率在CI组和健康对照组差异无统计学意义(P>0.05);CI组的PAI-1活性与对照组比较差异有统计学意义(P<0.01);PAI-14G/5G基因型多态性与PAI-1活性水平有关,3种基因型PAI-1活性间差异有统计学意义(P<0.01)。结论PAI-1基因4G/5G多态性与其血浆活性水平相关;4G等位基因者PAI-1活性水平增高,导致机体纤溶活性降低,从而增加CI的发病风险;PAI-1活性水平在CI发生过程中起重要的作用。

Abstract:Objective To evaluate the role of 4G/5G polymorphism of plasminogen activator inhibitor-1 promotor (PAI-1) in the development of cerebral infarction (CI) and the correlations between PAI-1 genotype and plasma PAI-1 activity in Chinese.Methods One hundred and thirty patients with CI were randomly recruited,with one hundred healthy subjects were enrolled as controls.PAI-1 genotypes (4G/4G,4G/5G and 5G/5G) were analyzed by sequence specific primers-polymerase chain reaction (PCR-SSP) technique.PAI-1 activity was determined by substrate chemoluminescent enzyme immunoassay.Triglyceride,total cholesterol,glucose were determined by biochemical analyzer.Results There were no significant differences in distribution frequency of PAI-1 genotypes and 4G and 5G allele genes between CI patients and controls (P>0.05); PAI-1 activity of CI group was significantly higher than that of controls (P<0.01).PAI-1 4G/5G polymorphism was associated with PAI-1 activity.There was significant difference in PAI-1 activity among 4G/4G,4G/5G and 5G/5G (P<0.01).Conclusion PAI-1 genotypes are related with PAI-1 plasma activity levels.Subjects with 4G allele gene have higher PAI-1 activity than that with 5G allele gene,which leads decrease of fibrinolysis activity and thus raise the risk for the attack of CI.PAI-1 activity levels play an important role in the development of CI.
