脑脊液耳鼻漏的诊断及手术治疗

目的：探讨脑脊液耳鼻漏的定性、定位诊断以及手术入路、修补材料及方法等问题，以提高手术治疗效果。方法：回顾性分析30例脑脊液耳鼻漏患者临床资料，并复习相关文献。结果：30例患者共进行43次手术，一次手术成功24例，6例经过多次手术。29例患者手术后脑脊液漏停止，1例失败，无感染及死亡。结论：脑脊液耳鼻漏患者必须经过一种甚至多种影像检查方法准确判断漏口位置；选择适合的手术入路以及采用多层严密的修补方法才能提高一次手术成功率。

Diagnosis and surgical treatment of otorhinorrhea

Objective:Discussing various diagnostic and localization methods,surgical approaches,mending material and surgical technique of otorhinorrhea for improving therapeutic efficacy. Methods: Thirty cases of otorhinorrhea operated on in our hospital were analyzed retrospectively and related literatures were rivewed. Results:Forty-three operations were carried out on 29 patients, in which 24 succeeded with first attempt,6 patients experienced twice or more operations. Twenty-nine patients were cured while 1 patient remained rhinorrhea at discharge.There was no infection complication. Conclusion:Multiple ways of image can be used to localize the CSF leaks accurately.Optimal approach and the surgical technique with adequate multilayer closure are important at first attempt.
