摘    要:目的:探讨经皮肾镜下气压弹道联合超声碎石清石系统治疗复杂性肾结石的疗效。方法:2006年11月~2008年5月,在B超定位或C型臂X光机透视定位,经皮肾镜下,运用气压弹道联合超声碎石清石系统治疗复杂性肾结石27例32侧。结果:25例一期碎石清石,其中5例双侧肾多发性结石同期行双侧碎石清石术,2例二期碎石清石;结石总清除率84.4%。平均手术时间128 min,平均住院时间9.5 d。无严重手术并发症。结论:气压弹道联合超声碎石清石系统治疗复杂性肾结石具有微创、高效、安全的特点,值得临床推广应用。

Abstract:Objective:To evaluate the clinical efficacy of combined pneumatic and ultrasound lithotripsy for complex renal calculi with percutaneous nephrolithotripsy.Methods:27 cases(32 laterals) with complex renal calculi underwent percutaneous nephrolithotripsy(PCNL)by ultrasonic power of Swiss Lithoclast Master(EMSIII,Switzerland)with positioning perspective of B-ultrasonography or C-arm X-ray machine during November 2006 and May 2008.Results:Primary removal of the calculi were achieved in 25 cases,among which 5 cases received simultaneous removal of bilateral multiple renal calculi and 2 cases were referred to secondary procedure.The curative rate was 84.4%(27/32)and average time consumption was 128min for the procedure.The hospital stay was 9.5 days and no serious complications occurred.Conclusion: Percutaneous nephrolithotomy with EMSⅢ for complex renal calculi is recommended for the minimal invasion,higher curative rate and safety in performance.
