通塞颗粒对老年COPD急性加重期炎性细胞因子的影响

目的 :探讨老年慢性阻塞性肺疾病 (COPD)急性加重期细胞因子的变化意义及通塞颗粒对其影响。方法 :COPD急性加重期 5 6例分为治疗组和对照组。治疗组在对照组治疗 (西医综合治疗 )的基础上 ,口服中药通塞颗粒。检测COPD治疗前后和健康老年人 2 0例血清粒 -巨噬细胞集落刺激因子 (GM -CSF)、IL - 1β、IL - 2、IL - 6、IL - 8水平。结果 :COPD急性加重期患者血清GM -CSF、IL - 1β、IL - 6、IL - 8均明显高于健康老年人 (P <0 0 1) ,IL- 2明显低于老年人 (P <0 0 1) ;治疗后 ,两组GM -CSF、IL - 1β、IL - 6、IL - 8水平均明显降低 (P <0 0 1,P <0 0 5 ) ,IL - 2水平明显增高 (P <0 0 1)。治疗组治疗后GM -CSF、IL - 1β、IL - 6、IL - 8水平的降低较对照组显著 (P <0 0 1) ,IL - 2水平升高较对照组明显 (P <0 0 1)。结论 :细胞因子分子的变化参与老年COPD急性加重期病理过程 ,中药通塞颗粒降低GM -CSF、IL - 1β、IL - 6、IL - 8水平 ,升高IL - 2水平可能是该方治疗老年COPD的主要机制之一。

Effect of Tongse Grain of TCM on Cytokine in Patients with Chronic Obstructive Pulmonary Disease

Objective :To explore the changes and significance on serum GM CSF,IL 1β,IL 2,IL 6,IL 8 in acute exacerbation patients with chronic obstructive pulmonary disease and the effect of Tongse grain of TCM composition on the item above. Methods :56 COPD patients with acute exacerbations treated with the routine therapy with Western Medicine as a therapy background.The patients were divided into treatment group(therapy with Tongse grain)and control group(routine therapy).It was observed that the level of serum GM CSF,IL 1β,IL 2,IL 6,IL 8 in the patients after and before treating and 20 healthy aged persons(HAP group). Results :The level of serum GM CSF,IL 1β,IL 6,IL 8 ncreased and IL 2 lowered obviously in the COPD patients with acute exacerbation comparwed with that in HAP group.Thwe level of serum GM CSF,IL 1β,IL 6,IL 8 in treatment group and control′s were significant lower after treating,the decrease of the item above in treatment group was obvious than that in the control′s. Conclusion :The changes of serum GM CSF,IL 1β,IL 2,IL 6,IL 8 were related to the development of COPD.The effect decreased the serum GM CSF,IL 1β,IL 6,IL 8 and increased of IL 2 might be one of mechanism Tongse grain of TCM′s treating acute exacerbation of COPD.
