股骨髁锁定钢板治疗股骨远端粉碎性骨折临床研究

[目的]探讨股骨髁锁定钢板治疗股骨远端粉碎性骨折的效果.[方法]2007年12月-2010年6月,使用锁定型股骨远端外侧解剖钢板治疗股骨远端严重粉碎性骨折27例.采用股骨髁锁定钢板进行手术治疗.[结果]手术时间65 ～150 min,平均116 min.术中出血量250～600ml,平均342 ml.未出现骨折劈裂、医源性神经血管损伤、骨筋膜室综合征及深静脉血栓等并发症.术后患者均获随访,随访16～44个月,平均21.6个月.X线片示22例骨折于术后24周愈合,3例36周愈合,2例48周愈合,平均27.1周.术后16～44个月,平均20.6个月取出内固定,经随访无再次骨折者.膝关节活动度121～135°,平均127°.随访期末膝关节HSS评分88～100,平均95.6,优良率100％.[结论]股骨髁锁定钢板治疗股骨远端粉碎性骨折疗效确切,是一种良好的治疗股骨远端骨折的方法.

Treatment of distal femur fracture by locking the femoral condyle with steel plate

[ Objective] To explore the effect of treating distal femur fractures by locking femoral condyle with steel plate. [ Method ] From December 2007 to June 2010, twenty-seven patients with distal femur fracture were treated with locking the femoral condyle with steel plate. [Results] The mean operation time was 116 minutes （ 65-150 minutes）and the mean blood loss was 342 ml （250-600 ml）. No fractures split off , no iatrogenic nerve and vascular injury occurred and no osteofaseial compartment syndromes or deep vein thrombus happened. Patients were followed fur 21.6 months on average（16-44 months）. X-ray showed 22 eases of fracture healed at 24 weeks after operation, 3 cases at 36 weeks to heal and two cases at 48 weeks to heal with an average of 27.1 weeks. The fixation was removed from 16 to 44 months, （average 20.6 months）. No more fracture occurred again. The mean range of knee motion was 127° （121° to 135° ） .The HSS score was 95.6 （88-100） on average at the end of the follow-up. The good rate was 100%. [Conclusion] Treating distal femur fractures with locking the femoral condyle by steel plate is a good amt effective method.
