血清环氧化酶-2和白介素-6水平在婴儿肺炎中的意义及相关性探讨

摘    要:目的探讨婴儿肺炎患者血清环氧化酶-2(COX-2)、白细胞介素-6(IL-6)的变化及临床意义。方法选择2007年10月~2008年5月在常州市第二人民医院住院治疗的婴儿肺炎急性期患者56例,其中男36例,女20例;年龄2~12月,纳入病例组;同期在上述医院进行体检的32例健康儿童作为对照组,差异无显著意义。采用酶联免疫吸附法((ELISA)检测治疗前和治疗后7天病例组和健康对照组血清标本中COX-2、IL-6的浓度并进行统计学分析。结果病例组治疗前血清COX-2(48.26±19.92)I U/L、IL-6(65.23±23.52)pg/mL较对照组COX-2(33.87±15.22)I U/L、IL-6(16.04±11.41)pg/mL明显升高;两者均有显著性差异(P<0.001)。病例组治疗7天后血清COX-2、IL-6水平与对照组相比无显著性差异(P>0.05)。血清COX-2与IL-6水平变化有统计学正相关性(P<0.001)。结论COX-2、IL-6可能参与了婴儿肺炎的发病过程,动态检测它们的含量,对于评估肺部感染是否被控制有一定的临床意义。

Significance of and correlation between the levels of serum COX-2 and IL-6 in infant pheumonia
Abstract:Objective The aim of the study was to elucidate the levels and clinical significance between cyclooxygenase-2(COX-2) and Interleukin-6(IL-6) in infant with pneumonia.Methods Fifty six infants with pneumonia(male 36,female 20,ranging,from 2 to 36 months in age) in pediatric ward of the second People′s hospital of Changzhou from October 2007 to May 2008 were involoed in the study and 32 sex-and age-matched healthy children in pediatric clinic as controls.The serum samples were withdrawn for COX-2 and IL-6 during 7 days before the treatment and 7 days after the treatment.The above tests were performed by using enzyme-linked immunosorbent assay.The data were analyzed.Informed consent was obtained from all parents and their guardians, and the trial was approved by the ethics committee of the second people′s hospital of Changzhou.Results The levels of serum COX-2 and IL-6 before treatment were significantly higher(COX-2: 48.26±19.92 IU/L,IL-6: 65.23±23.52 pg/mL) and 7 days after treatment were significantly lower in infant with pneumonia as compared to control subjects(COX-2: 33.87±15.22 IU/L,IL-6: 16.04±11.41 pg/mL),P<0.001,respectively.A significant positive correlation was found between the plasma COX-2 and IL-6 levels in statistics(r=0.4693,P<0.001).Conclusion COX-2 and IL-6 may be involved in the pathogenesis of infant pneumonia.Monitoring the levels of serum COX-2 and IL-6 may be useful to the assessment about whether pulmonary infection is controlled.
