慢性硬脑膜下血肿者纤溶功能的临床研究

30例慢性硬脑膜下血肿(CSDH)手术前、手术后1周取静脉血,手术中取血肿液检测PLG、PL、PAI、t-PA、FG、FDP。发现术前血液中PLG、PAI、FG明显降低(P<0.01),t-PA、FDP显著增高(P<0.01)。术后PLG、PAl、t-PA恢复正常,血肿液中PLG明显降低;FDP显著增高(P<0.01)。表明CSDH血肿局部及全身都存在纤溶亢进。血肿清除后全身纤溶功能趋正常,提示纤溶亢进在CSDH形成中起重要作用。

The Clinic Studies of Fibrionlytic Stale on Chronic Subdural Hematoma

We examined the PLG, PL, PAI, t-PA, FG and FDP levels in venous blood or hematocele of 30 cases of Chronic Subdural Hematoma (CSDH) at different times (1 wk before operation and ] wk after operation for venous blood; during operation for hematocele) . It is found that there is a obvious reduction in the plasma levels of PLG, PAI and FG before operation (P< 0.01) while the t-PA and FDP levels got a significant rise (P < 0.01). After operation, the plasma levels of PLG, PAI and t-PA restored to normal levels. In the hematocele collected during operation, the PLG levels decreased obviously and FDP level rose significantly ( P < 0, 01). These results suggested that a hyperfibrionlytic state existed in both local hematoma and blood circulation. The general fibrinolytic function was back to normal after hematoma-excision; indicating the hyperfibrinolysis played a important role in the formation of CSDH.
