支原体肺炎患儿血清可溶性细胞间粘附分子-1及心肌酶谱联合检测的意义

摘    要:目的:探讨检测支原体肺炎(MPP)患儿血清可溶性细胞间粘附分子-1(sICAM-1)及心肌酶谱在急性期与恢复期的变化和临床意义。方法:检测60例MPP患儿在急性期与恢复期血清sICAM-1及心肌酶谱的水平,同时与60例健康体检儿的相应检测结果进行比较。结果:①MPP患儿急性期血清sICAM-1、心肌酶谱水平明显高于恢复期和正常对照组(P<0.01);②MPP患儿恢复期血清sICAM-1、心肌酶谱水平与对照组相比无统计学差异(P>0.05);③60例MPP患儿急性期出现心肌酶异常(生化指标1项以上升高)48例,占80.00%。其中乳酸脱氢酶增高29例,占48.33%;磷酸肌酸激酶增高21例,占35.00%;门冬氨酸转氨酶增高17例,占28.33%;肌酸激酶同工酶增高12例,占20.00%;④急性期心肌酶异常的48例MPP患儿血清sICAM-1水平均高于心肌酶正常的12例MPP患儿(P<0.01)。结论:MPP患儿急性期大多有不同程度的心肌细胞损害,急性期存在心肌损害的MPP患儿血清sICAM-1水平明显增高,故急性期联合检测血清sICAM-1及心肌酶谱的水平有利于早期发现、早期诊断MPP患儿的心肌损害,及时加用营养心肌的药物,以免引起心肌的永久性损害。

Significance of joint detection of serum soluble intercellular adhesion molecule-1 and myocardial enzymes in children with Mycoplasma Pneumoniae pneumonia
Abstract:Objective:To explore the changes and clinical significances of serum soluble intercellular adhesion molecule-1(sICAM-1) and myocardial enzymes in children with Mycoplasma Pneumoniae pneumonia(MPP) at acute phase and recovery phase.Methods:The serum levels of sICAM-1 and myocardial enzymes in 60 MPP children at acute phase and recovery phase were detected,then the results were compared with those of 60 children receiving health physical examination during the same time.Results:The serum levels of sICAM-1 and myocardial enzymes in MPP children at acute phase were statistically significantly higher than those at recovery phase and normal control group(P<0.01);there was no statistically significant difference in serum levels of sICAM-1 and myocardial enzymes between MPP children at recovery phase and normal control group(P>0.05);among 60 MPP children,48 children were found with abnormal myocardial enzymes(more than one index increased) at acute phase,accounting for 80.00%,29 children were found with increase of lactic dehydrogenase,accounting for 48.33%;21 children were found with increase of creatine phosphate kinase,accounting for 35.00%;17 children were found with increase of aspartate aminotransferase,accounting for 28.33%;12 children were found with increase of creatine kinase isoenzyme,accounting for 20.00%;the serum level of sICAM-1 in 48 MPP children with abnormal myocardial enzymes was statistically significantly higher than that in 12 MPP children with normal myocardial enzymes(P<0.01).Conclusion:Injury of myocardial cells in varying degrees exists in MPP children at acute phase,and the serum levels of sICAM-1 in MPP children with myocardial injury at acute phase increase significantly,so joint detection of serum sICAM-1 and myocardial enzymes in MPP children at acute phase is beneficial to early discovery and diagnosis of myocardial injury in MPP children,the drugs having nutritional effect on myocardium should be applied timely to avoid permanent damage to myocardium.
