紧急深静脉穿刺置管术在心肺复苏中的临床应用

目的：探讨紧急深静脉穿刺置管术在心肺复苏（cardiopulmonary resuscitation,CPR）中的临床应用价值。方法：随机将58例心脏骤停患者分为深静脉穿刺置管给药组（深静脉组）30例和常规外周静脉给药组（外周静脉组）28例,比较分析两组复苏效果。结果：两组在穿刺置管时间上差异无统计学意义（P〉0.05）,但深静脉组自主循环恢复（Return of spontaneous circulation,RSC）时间明显短于外周静脉组（P〈0.05）,初期复苏成功率和短期存活率也显著高于外周静脉组（P〈0.05）,深静脉组的肾上腺素用量明显少于外周静脉组（P〈0.05）。结论：与常规外周静脉给复苏药比较,CPR时紧急深静脉穿刺置管给药提高了初期复苏成功率。

Rapid central venous catheterization during cardiopulmonary resuscitation

Objective：To investigate the value of rapid central venous catheterization during cardiopulmonary resuscitation. Method.. Fifty-eight patients with cardiac arrest were randomly divided into central and peripheral in-travenous groups for drug administration. We compared the effect of two pathways for drug administration during CPR. Result：There was no significant difference in time of catheterization between the two groups. The RSC time in the central intravenous group was shorter and early success rate was significantly higher than that in the periph-eral intravenous group. The total dose of adrenaline in the central intravenous group was significantly less than that in the peripheral intravenous group. Conclusion： Central intravenous for drug administration increased the success rate of CPR at an early stage.
