微创小切口椎弓根钉棒系统内固定术治疗胸腰椎骨折

摘    要:目的探讨微创小切口椎弓根钉棒系统内固定术治疗胸腰椎骨折的可行性及疗效。方法 2009年月3月~2011年9月应用微创小切口椎弓根钉棒系统内固定术治疗胸腰椎骨折12例,其中单纯压缩性骨折8例,爆裂性骨折4例,均不伴神经损伤。结果本组手术时间56(38~69)min,术中出血量60(50~80)ml。术后患者腰部疼痛症状消失8例,缓解4例。术后3 d复查X线示骨折复位良好,内固定位置满意。椎体高度恢复,椎管面积基本达到正常。本组12例均获得随访,平均时间15(6~24)个月。骨折均愈合,愈合时间平均5.0(3.5~6.5)个月。X线及CT检查可见椎弓根钉位置均正确,无断钉现象。结论微创小切口椎弓根钉棒系统内固定术治疗胸腰椎骨折治疗效果满意,可将手术创伤减到最小。

Minimally Invasive Pedicle Screw Fixation for Thoracolumbar Fractures

Abstract:Objective To evaluate the feasibility and effect of minimally invasive pedicle screw fixa- tion in the treatment of thoracolumbar fractures. Methods The clinical data of 12 thoracolumbar fractures cases without nerve injuries admitted from March 2009 to September 2011 were reviewed, including 8 cases of compression fracture and 4 cases of blowout fracture. All cases were performed minimally invasive pedicle screw system fixation via small incisions. Results The average operative time was 56 minutes ( ranged from 38 to 69 minutes) , and the average intra-operative blood loss was 60 ml ( ranged from 50 to 80 ml). The symptom of lumbago was disappeared in 8 cases and improved in 4 cases. The X-ray examination showed fracture reduction and the location of internal fixation was satisfaction, the vertebral body height was restored, and spinal canal area was achieved normal. All patients were followed up for 6 to 24 months with an average of 15 months. All fractures were healing in 3.5 to 6.5 months with an average of 5 months. The X-ray and CT scanning showed the location of pedicle screws were achieved satisfactorily without breaken. Conclusion The minimally invasive pedicle screw fixation can achieve a satisfactory effect with minimally surgical trauma in the treatment of thoraeolumbar fractures.
