摘    要:目的探讨超声内镜(EUS)在胃癌术前分期中的应用价值。方法选择行胃癌切除术47例患者,术前两周内均行EUS检查,获取EUS检查的T/N分期结果,同时获取术后病理T/N分期结果,以术后病理结果为金标准,与术前EUS检查结果进行对照,以了解术前EUS检查在判断T/N分期的准确性。结果在胃癌浸润深度T分期上,EUS准确率为70.2%,其中T181.8%,T2 80%,T3 63.6%,T4 66.7%;在判断淋巴结转移N分期上,准确率为74.5%,其中N-77.8%,N+72.4%。结论EUS能进行准确的胃癌术前T、N分期,有助于胃癌综合治疗方案的制定。

Abstract:Objective To evaluate the value of endoscopic ultrasonography(EUS) in preoperative staging of gastric cancer.Methods Seventy-five patients who were confirmed with gastric cancer by pathology were preoperatively staged by EUS within two weeks before surgery.We collected T / N staging of EUS examination result,and the postoperative pathologic T / N staging result at the same time.With postoperative pathologic results as the gold standard,we compared the resutlts with preoperative EUS examination results,in order to evaluate the accuracy of EUS examination result in T / N staging.Results The overall accuracy rate of clinical staging of T subsets by EUS in assessing the depth of tumor infiltration was 70.2%,of which T1 was 81.8%,T2 80%,T3 63.6%,T4 66.7%,respectively.The overall accuracy rate of N subsets by EUS in identifying lymph node metastases was 74.5%,of which N-was 77.8%,N+72.4%.Conclusion EUS plays an important role in preoperative T and N staging of gastric cancer.
