GnRHa预处理对子宫动脉栓塞术治疗子宫肌瘤临床疗效的影响

目的评价GnRHa预处理对子宫动脉栓塞术治疗子宫肌瘤临床疗效及安全性的影响。方法选择2010年5月～2012年12月我院行UAE治疗的子宫肌瘤患者35例,根据UAE术前是否经GnRHa预处理分为GnRHa预处理组(n=20)及对照组(n=15),测量预处理组GnRHa预处理后血清激素水平的变化及两组UAE术后肌瘤体积的变化情况。结果 GnRHa处理后,预处理组血清激素E2、FSH及LH水平较治疗前显著性下降,差异具有统计学意义(P<0.05或P<0.01);GnRHa预处理组UAE术后子宫缩小率、最大肌瘤体积缩小率及平均肌瘤体积缩小率均较对照组高,差异具有统计学意义(P<0.01)。结论术前GnRHa预处理可有效安全地提高UAE治疗子宫肌瘤的疗效。

The Clinical Efficacy on Pretreatment of Gonadotropin Releasing Hormone Agonist in the Treatment of Uterine Fibroid by Uterine Artery Embolization

Objective To investigate the clinical efficacy and safety on pretreatment of gonadotropin re- leasing hormone agonist（GnRHa） in the treatment of uterine fibroid by uterine artery embolization （UAE）. Methods A total of 35 patients with uterine fibroids admitted from May 2010 to December 2012 were divided into GnRHa pretreatment group（ n = 20） and control group （n = 15 ） according to whether they took GnRHa be- fore UAE. Serum hormone levels were measured after GnRHa pretreatment in GnRHa pretreatment group, and myoma volume were also measured 3 months after UAE in both two groups. Results Serum E2, FSH and LH levels after GnRHa pretreatment were significantly lower than those before GnRHa pretreatment in the Gn- RHa pretreatment group（ P 〈 0.05 or P 〈 0.01 ）. The reduction rate of uterus volume, the largest myoma vol- ume and the average myoma volume in GnRHa pretreatment group were remarkablely higher than those in the control group, there were statistic differences between two groups （ P 〈 0.01 ）. Conclusion GnRHa pretreatment can increase the clinical efficacy effectively and safely in treating uterine fibroids by UAE.
