舒芬太尼对小儿腹腔镜手术后麻醉恢复质量的影响

目的:观察舒芬太尼对小儿腹腔镜手术后麻醉恢复质量的影响。方法:选择腹股沟斜疝、鞘膜积液施行择期腹腔镜手术治疗的患儿60例,分芬太尼组(F组)和舒芬太尼组(S组),每组30例。麻醉诱导时F组静脉注射芬太尼4μg/kg,S组静脉注射舒芬太尼0.5μg/kg,观察记录基础值(T1)、拔管前(T2)、拔管后1 min(T3)、拔管后3 min(T4)的收缩压(SBP)、心率(HR),记录自主呼吸恢复时间、苏醒时间、镇静躁动评分(SAS)。结果:F组内各时点的SBP差异有统计学意义(P<0.05),两组内的各时点HR及S组内的各时点SBP差异无统计学意义(P>0.05);两组患儿自主呼吸恢复时间、SAS差异无统计学意义(P>0.05)。S组苏醒时间短于F组,差异有统计学意义(P<0.05)。结论:舒芬太尼应用于小儿腹腔镜手术能有效地抑制气管拔管的应激反应,镇痛作用强,苏醒质量高。

Effect of sufentanil on recovery quality of anaesthesia after laparoscopic surgery in children

Objective:To observe the effect of sufentanil on recovery quality of anaesthesia after laparoscopic surgery in children.Methods:Sixty children with oblique inguinal hernia and hydrocele of tunica vaginalis who were treated with elective laparoscopic surgery were selected and divided into fentanyl group and sufentanil group,30 children in each group.When conducting anesthesia induction,the children in fentanyl group were treated with intravenous injection of fentanyl(4 μg/kg),while the children in sufentanil group were treated with sufentanil(0.5 μg/kg),the basic levels(T1),levels of systolic blood pressure(SBP) and heart rate(HR) before tube drawing(T2),at one minute after tube drawing(T3),at 3 minutes after tube drawing(T4) were observed and recorded;the recovery times of autonomous respiration,the awakening times,and sedation agitation scale(SAS) scores were recorded in the two groups.Results:There was statistically significant difference in SBP among different time points in fentanyl group(P<0.05),but there was no statistically significant difference in HR at different time points between the two groups,there was no statistically significant difference in SBP among different time points in sufentanil group(P>0.05);there was no statistically significant difference in recovery time of autonomous respiration and SAS between the two groups(P>0.05).The awakening time of children in sufentanil group was statistically significantly shorter than that in fentanyl group(P<0.05).Conclusion:Sufentanil can effectively suppress the stress reaction of tracheal extubation during laparoscopic surgery in children,the analgesic effect is strong,and the quality of recovery is high.
