椎体成形术术中骨水泥渗漏原因分析及防治策略

目的探讨椎体成形术术中骨水泥渗漏原因分析及防治策略。方法分析2007年10月—2012年12月在该科行PVP或PKP的125例椎体压缩骨折其中26例出现骨水泥渗漏患者的临床资料,分析骨水泥渗漏原因。结果骨水泥渗漏与很多因素相关,从术前的病椎情况评估到术中的穿刺、注射,适应证选择,熟练准确的手术操作,完善的影像学监测等。结论尽管椎体成形术的骨水泥渗漏等并发症会带来诸多问题,但应用在椎体压缩性骨折中具有立刻止痛、早下床活动,减少长期卧床带来其他并发症等明显优势,应该深入研究以便更广泛的推广应用。

Leakage of bone cement in vertebroplasty surgery cause analysis and prevention strategies

Objective To investigate the leakage of bone cement in vertebroplasty surgery.Methods From October 2007 to December 2012 PVP or PKP were performed on 125 cases of vertebral compression fractures,of which cement leakage occurred in 26 cases.Analysis was made of the reasons for the leakage of bone cement.Results Cement leakage was related to many factors,intraoperative puncture,injection,indication for preoperative vertebral assessment,skilled and accurate operative imaging monitoring.Conclusion Immediately relieve pain.Although the complications of vertebroplasty cement leakage will bring a lot of problems,but its application to vertebral compression fractures has the advantages of immediate pain relief,early ambulation,fewer complications,which should be further studied to promote the application.
