彩色多普勒超声在诊断先天性椎动脉变异中的应用

目的探讨彩色多普勒超声在诊断先天性椎动脉变异中的应用价值及临床意义。方法对2732例疑诊为椎动脉型颈椎病患者行彩色多普勒超声检查观测椎动脉管腔结构、血管走行、血流信号等变化。结果在可疑椎-基底动脉供血不足患者中,检出椎动脉先天性发育异常589例,异常率为21.6%。其中椎动脉起源异常6例,椎动脉颈段入横突孔位置高240例,椎动脉发育不良520例。结论彩色多普勒超声能清晰显示椎动脉颅外段的管壁管腔结构、血管走行及椎动脉内的血流信号,从而为临床诊断椎动脉型颈椎病提供依据。

The application of color Doppler ultrasonography in congenital variation of the vertebral artery

Objective To evaluate the Color Doppler Ultrasonography （CDU） in diagnosis of con- genital variation of the vertebral artery and clinical significance. Methods A total of 2732 cases with highly suspected as vertebrobasilar insufficiency were applied with CDU. The vascular morphological changes and hemodynamies of the vertebral artery were observed. Results In all cases, 589 cases of congenital abnomal vertebral artery were diagnosed and the abnormal rate was 21.6%. Of all, 6 eases were originally abnormal and 240 cases were abnormal with the entry position of vertebral artery into the foramen in transverse process of cervical vertebrae. Vertebral artery dysplasia were observed in 520 cases. Conclusion The study showed that CDU has the potential to display the vascular morphological changes and hemodynamics of the extracrani- al vertebral artery. The CDU could be help for evaluating congenital variation of the vertebral artery, which should be considered as routine clinical evaluation.
