破裂大脑中动脉瘤的显微手术治疗

目的 总结显微手术治疗破裂大脑中动脉瘤的适应证、手术入路要点及并发症防治.方法 回顾性分析显微手术治疗32例破裂大脑中动脉瘤患者的临床资料.结果 32例中,31例行动脉瘤夹闭,l例行动脉瘤包裹术.出院时GOS评判良好28例,差3例,死亡1例.术后随访31例患者3-36个月,恢复正常23例,轻残6例,重残2例.结论 充分的术前评估和选择合适的手术入路是成功治疗破裂大脑中动脉瘤的保证.

Microsurgery for ruptured middle cerebral artery aneurysms

Objective To summarize the experience in treating the ruptured middle cerebral artery aneurysms (MCAA) by microsurgery and explore the surgical indications, the technique points of the surgical approach as well as the prevention and treatment of complications in the patients with ruptured MCAA aneurysms. MethodThe clinical data of 32 patients with ruptured MCAA aneurysms treated by microsurgery were analyzed retrospectively. Results The aneurysms necks were successfully clipped in 31 patients, and the aneurysm was wrapped in 1. According to GOS, the curative effects were good in 28 patients,poor in 3 ,and 1 died. 23 were recovered well,6 were moderately disabled,2 were severely disabled during a following up from 3 to 36 months in 31 survivors. Conclusions Sufficient preoperative evaluation and suitable operative approach underlie the successful microsurgical treatment of middle cerebral artery aneurysms.
