过伸损伤类胫骨平台骨折的诊断与治疗

目的:探讨膝关节过伸暴力导致的胫骨平台骨折的诊断与治疗。方法:回顾2004年6月～2006年10月收治的胫骨平台骨折,我们根据其X线表现分析其受伤机制,将其分为过伸损伤类和非过伸损伤类胫骨平台骨折。其中过伸损伤类胫骨平台骨折共18例,均给予胫骨结节及胫骨远端双向牵扯引治疗。结果:18例均获得随访,随访时间最长1年零8个月,最短6个月,平均随访时间10个月。根据Rasmussen提出的胫骨髁部骨折膝关节功能评分标准,优2例,良14例,中2例,差0例。结论:胫骨平台骨折应根据其骨折X线表现,详细分析其受伤机制,明确诊断,对发生过伸损伤类胫骨平台骨折者,一般不需要经皮撬拔复位或手术治疗,仅行胫骨结节及胫骨远端双向牵扯引,即可获得满意效果。

Diagnosis and Treatment of Over-Extensile Injured Type Tibial Plateau Fractures

Objective:To study diagnosis and treatment of the tibial plateau fractures which are injured by over-extensile violence on knee.Methods:We retrospected all the tibial plateau fractures that we treated from June of 2004 to October of 2006,we analysed their injured mechanism in light of the X-ray and divided them into two groups:over-extensile injured type and non-overextensile injured type.In them there were 18 cases over-extensile injured type tibial plateau fractures,which were treated with tibial tubercle and tibial distance dual-directional traction.Rseult:18 cases were followed up 6 to 20 months(mean 10 months),according to Rasmussen's clinical evaluation standard of knee for tibial plateau fractures,we should diagnose correctly according to their X-ray and analyse their injured mechanism in particular.for over-extensile injured type tibial plateau fractures,we can acquire satisfied therapeutic effect only through tibial tubercle and tibial distance dual-directional traction,percutaneous prise reduction and operation are not needed universally.
