摘    要:目的 研究 nm2 3m RNA,CD44 s和 CD44 v6表达与结直肠腺癌发生、发展及转移的关系 .方法 应用组织芯片技术 ,通过原位杂交和免疫组织化学方法分别检测 40例结直肠腺癌标本和 2 2例正常结直肠组织标本中 nm2 3m RNA,CD44 s和 CD44 v6表达情况 .结果  nm2 3m RNA在结直肠腺癌组织和正常组织中表达无显著性差异 (P>0 .0 5 ) ,且与结直肠腺癌的分化程度无相关性 (P>0 .0 5 ) ,但与癌细胞的侵润及淋巴结转移呈明显负相关 (r=- 0 .49,P<0 .0 1) .CD44 s在结直肠腺癌组织和正常组织中阳性率分别为 42 %(17/ 40 )和 18. 1%(4 / 2 2 ) ,有显著性差异 (P <0 .0 5 ) ,而CD44 v6只在结直肠腺癌组织中表达 ,阳性率为 5 5 . 8%(2 2 / 40 ) ,且与结直肠腺癌淋巴结转移和癌侵润呈明显正相关(r=0 .47,P<0 .0 1) .在高、中分化腺癌组织中 ,CD44 v6阳性率分别为 45 .8%(11/ 2 4)和 6 8.7%(11/ 16 ) ,有显著性差异(P<0 .0 5 ) ,但在直肠腺癌组织与结肠腺癌组织间表达无显著性差别 (P>0 .0 5 ) .结论  CD44 s、CD44 v6在结直肠腺癌组织中过量表达伴随 nm 2 3m RNA表达缺失 ,与结直肠腺癌转移和侵润密切相关 ,联合检测 3种基因能更准确判断结直肠腺癌转移和侵润状态 . CD44 v6表达过量与结直肠腺癌分化程度相关 .

Abstract:AIM To study the relationship between the expression level of nm23 mRNA, CD44s and CD44v6 and the biological behavior of human colorectal adenocarcinoma. METHODS Using tissue microarray technique, specimens of 40 colorectal adenocarcinoma and 22 normal colorectal mucosa were examined for CD44s, CD44v6 and nm23 mRNA expression by immunohistochemical (IHC) staining and in situ hybridization (ISH). RESULTS Expression of nm23 mRNA in the tissues of colorectal adenocarcinoma differed little from that in normal colorectal mucosa (P>0.05), nor did it correlate with pathological grade of the colorectal adenocarcinoma (P>0.05). But it was negatively related to the invasion of tumor and lymph node metastasis (r=-0.49, P<0.01). The expression of CD44s in the tissues of colorectal adenocarcinoma and normal colorectal mucosa was 42% (17/40) and 18.1% (4/22) respectively and they had statistical difference (P<0.05). But the positive staining of CD44v6 was only seen in the tissues of colorectal adenocarcinoma (55.8%, 22/40) and they were positively related to lymph node metastasis and invasion (r=0.47, P<0.01). The expression of CD44v6 in the high and middle differentiated colorectal cancer were 45.8% (11/24) and 68.7% (11/16) respectively, which had significant difference (P<0.05), but which had no striking difference from the expression in human colorectal adenocarcinoma (P>0.05). CONCLUSION Patients who had overexpression of CD44s and CD44v6 and low expression of nm23 mRNA have a higher invasion of tumor and lymph node metastatic rate. Combined detection of three genes is a reliable index to evaluate the invasion and metastasis of colorectal adenocarcinoma. In addition, overexpression of CD44v6 correlates with the pathological grade of colorectal adenocarcinoma.
