生物节律基因Timeless表达与不同部位直肠癌预后的关系

摘    要:目的分析不同部位直肠癌生物节律基因Timeless的表达及与预后的关系,为患者个性化治疗及预后监测提供参考。 方法选取2012年4月至2015年4月淮安市第一人民医院分院收治的384例直肠癌患者作为样本来源,根据肿瘤位置不同分为高位组(n=64)、中位组(n=122)和低位组(n=198)。分别采用实时荧光定量PCR和免疫组织化学检测各组癌组织和癌旁正常组织中Timeless mRNA及蛋白表达水平,并分析其与临床特点及生存情况的关系。 结果高、中和低位癌三组直肠癌组织Timeless mRNA及蛋白表达水平均高于癌旁正常组织,差异均有统计学意义(P<0.01);三组间癌组织Timeless mRNA及蛋白表达水平比较,表现为:低位组>中位组>高位组(P<0.05)。Timeless mRNA及蛋白在肿瘤大小、分化情况、TNM分期及生存情况上的表达差异有统计学意义(P<0.05)。Cox回归分析显示,肿瘤位置、TNM分期及Timeless表达是影响直肠癌预后的独立危险因素(P<0.05)。 结论Timeless在直肠癌中高表达,尤其以低位直肠癌最明显,其表达程度与肿瘤大小、分化情况、TNM分期及生存情况密切相关,是影响其预后的独立危险因素之一。

Abstract:ObjectiveTo analyze the expression of biorhythmic gene Timeless in different parts of rectal cancer and their relationship with prognosis, so as to provide more possibilities for individualized treatment and prognosis monitoring. MethodsFrom April 2012 to April 2015, 384 patients with rectal cancer admitted to Branch Courts of Huai’an First People’s Hospital were selected as source of specimens. According to the location of the tumors, they were divided into high group (n=64), middle group (n=122) and low group (n=198). The Real-time fluorescence quantitative PCR and immunohistochemistry were used to detect the expression of Timeless mRNA and protein in cancer tissues and adjacent normal tissues, and to analyze their relationship with clinical characteristics and survival. ResultsTimeless mRNA and protein in cancer tissues of high, middle and low rectal group were higher than those in normal adjacent tissues (P<0.01). The difference of Timeless mRNA and protein among the three groups was statistically significant (P<0.05), which was low group > middle group > high group. There were differences in tumor size, differentiation, TNM stage and survival in Timeless mRNA and protein. Cox regression analysis showed that tumor location, TNM stage and Timeless were independent risk factors for prognosis of rectal cancer (P<0.05). ConclusionTimeless is highly expressed in rectal cancer, especially in low rectal cancer, which is closely related to tumor size, differentiation, TNM stage and survival, and is one of the independent risk factors affecting its prognosis.
