C反应蛋白在评估新生儿早发感染诊断中的价值

目的评估新生儿早发感染中C反应蛋白的诊断价值。方法前瞻性、观察性研究,将110例怀疑早发感染的住院新生儿随机分为感染组44例和非感染组66例,比较两组间CRP水平、血常规结果。结果感染组与非感染组在白细胞计数异常、中性细胞绝对计数异常、血小板计数异常构成比及白细胞计数、中性分类、中性细胞绝对计数、血小板计数水平比较差异均无统计学意义,而感染组CRP水平明显升高,与非感染组比较,Z=-4.858,P=0.000,两组之间差异有统计学意义;CRP在新生儿早发感染诊断的AUC为0.758（0.664～0.853）,CRP≥10mg/L时,敏感度40.9%、特异度93.9%。结论新生儿早发感染中CRP具有一定诊断价值,特异度较高,但敏感度较低。

The significance of C-reactive protein for diagnosis of early onset infection in newborn infants

Objective To evaluate the accuracy of C-reactive protein for diagnosis of early onset infection in newborn infants.Prospective and observational study.110 neonatal cases with suspected infection were enrolled.Patients were classified into infectious groups（n=44）and no-infectious group（n=66）based on clinical progress and culture results,blood samples for determining whole blood CRP.The medians for CRP was statistical difference between the groups,the area under the receiver operating characteristic （ROC） curve （AUC） for CRP was 0.758,for the cut-off value of CRP≥10 mg/L,the test was found to have a sensitivity of 40.9%,specificity of 93.9%.CRP levels was useful for the diagnosis of early onset infection in newborn infants.
