676例肝炎肝硬化并发自发性细菌性腹膜炎分析

目的 为了探讨肝炎肝硬化并发自发性细菌性腹膜炎(SBP)的危险因素及控制措施,对我科1996.8～1999.12收治的676例肝硬化患者并发SBP及其危险因素进行回顾性分析。结果显示,肝硬化并发SBP38例(5.62％);在41例次SBP中,Child-PughC级约占75.61％;其中低蛋白血症、脾功能亢进、肝性脑病、消化道出血等患者的发病率较高(分别为7.84％;8.68％;47.83％;41.46％);16例次阳性菌中,G~-细菌10株,G~+2株,霉菌4株。结果表明,SBP的危险因素与患者住院时间长短、血浆蛋白水平、脾功能亢进以及是否合并肝性脑病和消化道出血等因素密切相关。

RISK FACTORS OF SPONTANEOUS BACTERIAL PERITONITIS IN PATIENTS WITH LIVER CIRRHOSIS

In order to investigate risk factors and measures controlling the hospital infection of spontaneous bacterial peritonitis (SBP) in patients with liver cirrhosis. Methods The risk factors of SBP in patients with liver cirrhosis were studied by investigation of 676 LC patients. Results SBP rate was about 5. 62% ,and frequency rate 6. 07%. Of 41 SBP occasions, the rate of Child-Pugh C grade was about 75. 61%. Several a-gents.such as low plasma albumin content,hypersplenism,hepatic encephalopathy,and hemorrhage of digestive tract,had higher infection rate (7. 84%,8. 68%,47. 83%, respectively). Of 16 positive strains of bacterial culture, 10 were gram-negative bacterium,2 were gram-positive bacterium,and 4 were mycete. Conclusion The risk factors for SBP in patients with liver Cirrhosis may be associated with patients duration of hospitalization, low plasma albumin content,hypersplenism,hepatic encephalopathy,and hemorrhage of digestive tract.
