外科手术患者序贯用药安全性的讨论

目的:了解近年外科手术患者用药变化情况,探讨术后镇痛泵(PCA)应用带来的新问题与序贯用药的安全性。方法:收集本院临床203例患者手术当日的用药资料、对不同专科用药以及PCA与序贯(品种依次累计)用药安全性问题进行调查分析。结果:本组患者累计共用药物88个品种1746种／次,人均用药8．6种／次,49．26％的患者术后应用了PCA,不同专科在使用麻醉药、镇痛镇静药及其他类药物的结果经U检验处理,均有统计学意义(P<0．01)。结论:近年来外科手术情况与药物的应用均出现了一些新变化,本组用药存在显著的专科特点,PCA的引用增加了外科手术序贯用药潜在的不安全因素。应加强对外科手术患者序贯用药安全性的监护,提高其用药的合理性。

Discussion the Safety of Sequentially Drug Using in Surgical Patients

Objective: To study on the use of patient-controlled analgesia (PCA) and discussion the safety of sequentially drug using. Method: The patients with operation drugs use on the operative day were collected and analysed in 203 cases, drugs use in dilferent department were compared, and the safety of sequentially drug using was particularly investigated. Result: 88 kinds of drugs were used 1746 times, 8.6 kinds or times of drug were administrated to a patient averagely. 49. 26% patients after operation used PCA . There was difference between departments in the use of anesthetics, analgesics and sedatives( P <0. 01). Conclusion: There are several changes in the methods and drug use in operation, and there are marked differences between departments. Administration of PCA de-creas the safety of sequentially drug using after operation, therapeutic responses must be monitored carefully.
