非瓣膜性心脏病心房颤动的抗凝治疗进展

非瓣膜性心脏病心房颤动(NVAF)是指风湿性心脏病以外的其他心脏病所致的心房颤动。随着社会的老龄化,NVAF逐年增多,栓塞和卒中的发生率也随之升高。其治疗主要存在三大难题:一是心房颤动复律与窦律的维持;二是心室律的有效控制,是对不能复律者的有效控制;三是防止发生卒中。本文就NVAF的流行病学特征、栓塞和卒中的危险因素,华法令在抗凝治疗中的临床应用、局限性、出血并发症及新型口服直接凝血酶抑制剂的出现和应用前景,中药在抑制血小板聚集、改善纤溶能力、减少或延缓血栓的形成中的抗凝治疗作用等问题进行综述。
