以阳性症状为主慢性精神分裂症利醅酮治疗前后EEM与ERP对照研究

目的探讨探究性眼动分析和事件相关电位评价以阳性症状为主的慢性精神分裂症患者的诊断和治疗效果的应用价值。方法将42例以阳性症状为主的慢性精神分裂症患者设为研究组，口服利培酮治疗，观察6周；随机抽取40例正常健康者设为对照组。于研究组治疗前及治疗6周末测评两组探究性眼动分析和事件相关电位，并进行对比分析。结果研究组治疗后眼球凝视点数与反应性探索分均较治疗前显著升高（P〈0．01）；事件相关电位的P300及N400潜伏期均较治疗前显著缩短（P〈0．01），波幅均较治疗前显著升高（P〈0．01）。研究组治疗前后眼球凝视点数与反应性探索分均显著低于对照组（P＜0．01），事件相关电位的P300及N400潜伏期均较对照组延长（P〈0．01），波幅均较对照组显著下降（P〈0．01）。结论探究性跟动分析和事件相关电位测评可作为评价慢性阳性精神分裂症患者诊断和疗效判定的客观依据，但可能受药物治疗影响。

A control study of EEM and ERP before and after risperidone treatment for chronic schizophrenia patients with positive symptoms

Objective To explore the value of exploratory eye movements （EEM） and event-related potential （ERP） evaluating diagnosis and curative effect of the chronic schizophrenia patients with positive symptoms. Methods Forty-two chronic schizophrenics with positive symptoms were assigned to research group taking orally risperidone for 6 weeks and randomly selected 40 normal healthy persons to control group. The EEM and ERP mensurations were conducted and contrastivele analyzed in the 2 groups before treatment and at the end of the 6th week. Results After treatment the number of eye fixation（NEF） and responsive search score（RSS） advanced compared with pretreatment in the research group （P〈0.01）; Both P300 and N400 latency of the ERP shortened significantly（P〈0.01） and amplitude heightened significantly （P〈0.01）. Both NEF and RSS were significantly lower before and after treatment（P〈0.01）, both P300 and N400 latency longer（P〈0.01） and amplitude lowered more significantly in the research than in the control group （P〈0.01）. Conclusion EEM and ERP may be regarded as objective evidences for diagnosis and curative effects of the chronic schizophrenia patients with positive symptoms, but be influenced by medication.
