早期灌肠对减轻早产儿黄疸的临床研究

目的：探讨早期灌肠对减轻早产儿黄疸的临床效果。方法将45例早产儿随机分为灌肠组22例，对照组23例。对照组采取常规治疗和护理，灌肠组在给予常规治疗和护理的基础上，出生24 h内给予温盐水加开塞露灌肠。观察两组早产儿光疗天数、黄疸持续天数及首次胎便转黄时间、每日排黄便次数。结果灌肠组早产儿的光疗天数、黄疸持续天数较对照组短，差异有显著意义（P＜0．05）；灌肠组早产儿首次胎便转黄时间较对照组早、灌肠组每日排黄便次数较对照组多，差异有显著意义（P＜0．05）。结论对早产儿实施早期灌肠，可缩短光疗时间、胎粪较早排尽，减轻黄疸程度、降低病理性黄疸的发生率，提高早产儿生存率及生存质量。

Clinical study for the effect of early enema on jaundice for premature infants

Objective To study the clinical effect of early enema on jaundice for premature infants .Method 45 premature infants were selected and randomly divided into two group ,22 cases in enema group ,23 cases in control group .Infants in enema group was given warm saline and glycerine enema for intervention based on the routine treatment and nursing within 24 hours after born .infant in control group was given conventional therapy and nurs-ing care .The following indicators were observed in the two groups ,such as jaundice phototherapy time duration and the first meconium turning yellow ,daily defecation times .Result The phototherapy days and jaundice duration of the enema group were shorter than that of control group .There was significant difference between groups（P〈0 .05） . The first meconium turning yellow time of the enema group was earlier than that of the control group .The daily def-ecation times of the enema group was more compared with the control group .There was significant difference be-tween groups（P〈0 .05） Conclusion Early enema can shorten the time of phototherapy ,earlier drained meconium ,re-duce jaundice ,reduce the incidence of pathologic jaundice rate ,improve the survival rate and the quality of life in premature infants .
