开颅术后昏迷患者中枢性呃逆的临床处理

目的 :探讨开颅术后昏迷患者中枢性呃逆的临床处理方法。方法 :对 2 3例患者发生中枢性呃逆后 ,即时均给予吸痰、给氧、压迫人中穴位等对症处理 ;长期治疗上分为甲乙两组 ,甲组 11例患者给予由光子氧透射治疗仪静脉输氧处理 ,每次 2 5 0ml,持续 1h ,隔日 1次 ,连续 7次。乙组 12例 ,除上述静脉输氧治疗外 ,联合使用中药瓜蒂汤共 3剂 ,每日 1剂 ,经鼻胃管喂饲。甲乙两组临床资料采取卡方检验比较。结果 :顽固中枢性呃逆的昏迷患者中 15例 (6 5 .1% )在作吸痰、给氧处理后 ,其症状得到了有效缓解 ,但不久呃逆症状再次发作。长期治疗甲组治愈 5例 ,有效 2例 ,无效 4例。乙组治愈 10例 ,有效 2例 ,无效 0例 ;乙组疗效明显优于甲组 ,P <0 .0 5 ,差异有显著性。结论 :痰液对呼吸道梗阻造成的缺氧是神经外科中枢性呃逆患者的重要影响因素之一 ,静脉输氧和中药瓜蒂汤治疗中枢性呃逆有效。

The Clinical Treatment of the Central Hiccup in the Patients of Post-operation on Brain

Objective: To study the ways of curing the central hiccup in the patients of post operation on the brain. Methods: We divided 23 patients into two groups. The first group had 11 patients,which were sent oxygen by dropping in vein;the second group had 12 patients,which besides sent oxygen in vein,were given Chinese medicine Guadi soup. And compare the curative effect of the first team and the second team. Results: In the first group 5 cases were mended,2 cases turned better and 4 cases didn't change. In the second group 10 cases were mended,2 cases turned better. Conclusion: The combination of hinese medicine and the way of sending oxygen by dropping in vein is much better than sending oxygen in vein merely.
