西药促排卵效果与中医证型的相关性探讨

目的研究排卵障碍性不孕症患者不同中医证型与西药促排卵治疗效果的相关性,并探讨其临床意义。方法对临床确诊为排卵障碍性不孕症患者80例进行中医辨证分型,所有患者予以克罗米芬促排卵治疗,连续5 d,并对治疗后的排卵及妊娠结局进行分析比较。结果各证型组患者治疗后排卵率及妊娠率从高到低依次为：肾阳亏虚型（85.00%,20.00%）、肾阴亏虚型（78.57%,14.29%）、痰湿内阻型（75.00%,12.50%）、肝气郁滞型（50.00%,11.11%）和瘀滞胞宫型（75.00%,8.33%）,其中排卵率肾阳亏虚型、肾阴亏虚型优于痰湿内阻型、瘀滞胞宫型以及肝气郁滞型（P〈0.05）。结论排卵障碍性不孕症中医各个证型的排卵率及妊娠率不同,提示各个证型患者生殖内分泌或代谢异常程度有所差异,对西药促排卵的效果也有所差异。

Correlation between Ovulation Induction Effect and Chinese Medicine Syndrome Types

Objective To study the correlation of Chinese medicine syndrome types of anovulatory infertility and ovulation induction effect,and explore its clinical significance.Methods Eighty patients with anovulatory infertility were syndrome differentiated as five types,and were given clomiphene as the ovulation induction for 5 successive days.Results of ovulation and pregnancy were analysed and compared.Results Ovulation rate and pregnancy rate from high to low were as follows:Kidney yang deficiency syndrome(85.00%,20.00%),kidney yin deficiency syndrome(78.57%,14.29%),phlegm dampness accumulation syndrome(75.00%,12.50%),liver qi stagnation syndrome(50.00%,11.11%),and blood stasis in uterus syndrome(75.00%,8.33%).Significant difference was shown in ovulation rate of kidney yang deficiency syndrome and kidney yin deficiency syndrome compared with phlegm dampness accumulation syndrome,blood stasis in uterus syndrome and liver qi stagnation syndrome(P<0.05).Conclusion Different Chinese medicine syndrome types of anovulatory infertility can show different ovulation rate and pregnancy rate.The effects of ovulation induction in patients were different,which indicates the reproductive endocrine abnormality or the metabolism abnormality.
