心脏手术后医院感染经济损失的前瞻性队列研究

目的评估心脏手术患者发生医院感染所造成的经济学损失。方法采用前瞻性队列研究方法 ,依据是否发生医院感染,将2306例接受心脏手术的患者分为感染组和未感染组,比较两组的住院日和医疗费用。结果感染组患者的中位住院天数为46.76d,医疗总费用、药费、检查费和治疗费分别为246788.83、79732.39、10704.31、38299.33元,均明显高于未感染组的81073.14、13150.75、2760.74、5808.43元,差异有统计学意义(P0.01)。结论医院感染可延长心脏手术患者的住院时间,增加其医疗费用,应当采取有效措施控制医院感染。

Economic Losses due to Nosocomial Infections in Patients after Cardiac Surgery:A Prospective Cohort Study

OBJECTIVE To evaluate the economic losses due to nosocomial infections(NI)in patients after cardiac surgery.METHODS Totally 2306 patients after cardiac surgery have been divided into NI group and non-NI group.We used prospective cohort study to compare the median hospital stay and the medical expenses between the two groups.RESULTS The median hospital stay,the total medical expenses,the medications expenses,the examination expenses,and the treatment expenses were 46.76 days,246788.83 yuan RMB,79732.39 yuan,10704.31yuan,and 38299.33 yuan,respectively in NI group,and they were significantly higher than those Were 24.35 days,81073.14 yuan,13150.75 yuan,2760.74 yuan,and 5808.43 yuan,respectively in non-NI group(P<0.01).CONCLUSIONS NI will prolong hospiral stay and increase medical expenses of patients after cardiac surgery.Effective measures should be taken to control and eventually decrease the economic burden of patients.
