贵州省贵定县乐邦乡严重骨畸形病病例调查研究

摘    要:对贵州省贵定县乐邦乡村民罹患以骨骼严重畸形为特征的疾病病例研究结果显示,病家生活燃煤,敞灶烘烤之玉米,辣椒及室内贮存之谷子铝和氟含量显著高于对照。患者尿中铝和氟含量亦较对照高。28例骨畸形病人中仅有21例患轻或中度氟斑牙。病人骨X光片呈骨软化症,无氟骨症骨周组织异常骨化。三例严重骨畸形病人髂骨活检,现人骨铝含量显著增高,但骨氟含量无明显增加,骨形态计量学测量显示,病人骨损特征为骨发育障碍,成骨过程

Abstract:tudies were conducted on the patients with severe bone deformities in the area of Lebang,Guidin coun-ty,Guizhou Province.The results showed that the contents of aluminum(Al)and fluorine(F)in the coal,inthe corn(maize)baked by the coal smoke and in the rice(with the husks ) stored in the homes of patientswere significantly higher than those of control. The levels of Al and F in the urine of ptients were higher com-pared with those of control. 21 subjects out of 28 patients with bone deformities presented with mild or medi-um degree of dental fluorosis.Radiography of bones showed osteoporosis and osteomalacia without calcifica-tions of ligaments,irregular periosteal bone formation and calcification of interosseous membrane that are reg-ular findings seen in skelatal fluorosis. The content of Al in iliac crest biopsy specimens from patients withbone deformities was greatly higher,but the content of F was not evidently increased compared with control.Bone histomorphometry showed osteoporosis and osteomalacia with decresed mineralized bone formation andincreased osteoid volume. The specimens from patients showed a positive Al staining which chiefly exists be-tween mineralized bone and osteoid (mineralized front).The evidences indicate that occurence of osteoporosisand ostepmalacia in the patients in the area of Lebang are mostly related to Al intoxication induced by the ac-cumulation of Al in the bone.Since 75%of patients suffered dental fluorosis,the disease is called endemic Aland F intoxication.
