异丙酚与氯胺酮静脉复合麻醉在小儿手术中的应用

摘    要:目的 对比观察异丙酚复合氯胺酮静脉滴注给药和单用异丙酚静脉给药用于小儿麻醉的临床效果.方法 选择小儿手术麻醉82例,随机分为观察组和对照组,每组4 例.观察组采用异丙酚与氟胺酮复合麻醉,对照组单用异丙酚静脉麻醉.记录两组手术时间、异丙酚的用量,观察术前、切皮时、探查时、缝合时的心率(HR)、平均动脉压(MAP)、血氧饱和度(Sp02)的变化,手术过程中的不良反应及苏醒时间.结果 观察组异丙酚每公斤体重的平均用量明显少于对照组(P<0.001).观察组的手术时间及苏醒时间均明显短于对照组(P<0.05或P<0.001).术中心率、血压和血氧饱和度的变化对照组较观察组明显(P<0.05或P<0.001).结论 异丙酚与氯胺酮静脉复合麻醉用于小儿手术安全有效,麻醉过程平衡,优于单用异丙酚静脉给药.

Abstract:Objective To compare the effect of child propofol-ketamine combination intravenously anesthesia with intravenously propofol alone anesthesia. Methods Eightytwo child were randomly divided into bservation group (propofol-ketamine combination intravenous anesthesia, n=41) and control group (intravenous propofol alone anesthesia, n=41). The operating time and the dose of propofol of two groups were recorded respectively. The change of HR, MAP, SpO2 before operation and during skin incision, exploration and skin sutures after medicine, and the side effect during operation and recovery time were observed. Result The dose of observation group were less than those of control group (P <0.001). The operating time and recovery time in observation group were shorter than those of control group (P<0.05 or P <0.001) The changes of HR, MAP , SpO2 during operation in observation group were obviously than those in control group (P <0.05 or P <0.001). Conclusion Propofol-ketamine combination intravenous anesthesia in children is safe and effective, and it is better than the method of intravenous propofol alone anesthesia.
