眶上锁孔入路显微手术治疗前交通动脉瘤

目的探讨经眶上锁孔入路治疗前交通动脉瘤的手术方法和治疗效果。方法对28例前交通动脉瘤采用眶上额外侧锁孔入路技术，开放相关脑池，显露前交通动脉复合体并夹闭前交通动脉瘤，术中均经神经内镜观察证实夹闭满意。17例于术后近期做影像学复查。结果28例前交通动脉瘤全部一次成功夹闭；其中4例术中有动脉瘤破裂。无手术死亡及严重的手术并发症。随访28例全部恢复正常生活与工作。结论应用眶上锁孔入路技术手术治疗前交通动脉瘤较常规手术更精细、准确、安全、微创。

Microsurgical Treatment of Anterior Communicating Artery Aneurysms via Supraorbital Keyhole Approach

Objective To discuss the microsurgical treatment of anterior communicating artery aneurysms(ACAA) via the supraorbital keyhole approach.Methods Through a lateral supraorbital frontal keyhole approach,related cisterna was opened in 28 cases of ACAA to expose anterior communicating artery complex and ACAA were clipped.During the operation,the clipping was determined to be satisfied under the neuroendoscopy.Seventeen cases of ACAA were subjected to the visual reexamination after operation.Results All 28 patients with ACAA were clipped successfully.Intraoperative aneurysm rupture occurred in 4 patients.There was no operative death or severe operative complications.All 28 patients have returned their normal livings and work through a follow-up survey.Conclusion The surgical treatment of ACAA via supraorbital keyhole approach is more refined,accurate,safe and minimally invasive than the regular operations.
