三叉神经痛显微血管减压术中岩静脉的保护及意义

目的探讨岩静脉在三叉神经手术中的保护及其临床意义。方法回顾性分析三叉神经痛微血管减压术（MVD）285例，其中206例岩静脉被电凝处理，79例保护良好。结果79例岩静脉保护良好的术后未发生脑水肿，无明显恶心呕吐。206例电凝处理岩静脉的其中3例发生小脑水肿，2例经脱水剂治疗，1例经后颅窝减压后恢复良好，35例术后3天明显恶心呕吐，腰穿测脑压高。结论岩静脉在三叉神经血管减压手术中应尽可能保护良好，否则术后可常规应用脱水剂以及做好再次后颅窝减压手术的准备。

Significance and protection of petrosal vein in microvascular decompression for trigeminal neuralgia

Objective To discuss the preservation of petrosal vein in the operation of trigeminal neuralgia. Methods The clinical data of 285 patients with trigeminal neuralgia were underwent by microvascular depression（MVD） , in whom the petrosal vein were protected in 79 and failed to be protected among 206, were analyzed retrospectively. Results There was no cerebral edema, nausea and vomitting in 79 cases with intact petrosal vein. The cerebral edema in 3 cases, nausea and vomitting in 35 were observed in 206 with petrosal vein damage. They recovered well after decompression of posterior cranial fossa （ 1 case） and dehydration therapy （2 cases）. Nausea and vomitting 3 days after operation in 35 cases and brain pressure was high of lumbar puncture. Conclusions Petrosal vein should be well protected in the operation of trigeminal neuralgia. The decompression of posterior cranial fossa should be considered after dehydration therapy if the petrosal vein is failed to be protected.
