缓释盐酸二氢奎尼丁预防阵发性房颤的疗效观察

目的观察缓释盐酸二氢奎尼丁（赛利科）预防中、老年人阵发性房颤（ＰＡＦ）复发的疗效、有效药物剂量及副作用。方法对６４例中、老年ＰＡＦ患者随机应用赛利科和乙胺碘呋酮治疗及预防复发，并随访１～３年。资料中各种数据的统计学处理，组内比较采用ｔ检验，组间比较采用卡方检验。结果赛利科和乙胺碘呋酮预防ＰＡＦ的有效率分别为７６．５％和７０％，无显著差异（Ｐ＞０．０５）。乙胺碘呋酮组服药前后的心率、ＳＮＲＴ和ＳＡＣＴ的变化均有显著差异（Ｐ＜０．０５）；而赛利科组则无显著差异（Ｐ＞０．０５）。赛利科预防ＰＡＦ的有效剂量在６００ｍｇ·ｄ－１以下者２３例，占６８％；２例用量在９００ｍｇ·ｄ－１，占６％；均无明显副作用。用量达１２００ｍｇ·ｄ－１者５例中２例产生明显副作用。结论赛利科预防ＰＡＦ复发的疗效与乙胺碘呋酮相似，但前者对窦房结抑制作用较后者轻。有效药物剂量大部分患者在６００ｍｇ·ｄ－１以下即可取得良好疗效。超过９００ｍｇ·ｄ－１者副作用明显增加，疗效并不增加

Clinical Observation of Serecor in Preventing Paroxysmal Atrial Fibrillation

Objective To study the effect,effective dosage and side actions of Serecor in preventing paroxysmal atrial fibrillation(PAF) in middle and old aged patients.Method Sixty four patients with PAF were randomly divided into two groups.The first group was treated with Serecor.The second group was treated with Amiodarone.The patients were followed up for 1～3 years.The data were statistically dealt with by t or χ 2 test.Results The effective rates of Serecor and Amiodarone in preventing PAF are 76.5% and 70% respectively,and are not statistically different( P >0.05).The changes of heart rate,sinus nods recovery time and sino atrial conduct time of the patients treated with Amiodarone before and after the therapy were significant( P <0.05),but the changes of the patients treated with Serecor were not significant( P >0.05).The effective dosage of Serecor is 600mg·d -1 or less in 23 cases(68%),900mg·d -1 in 2 cases(6%),in whom the side actions are few.In two of five patients with 1200mg·d -1 ,the side actions are significant.Conclusions The effect of Serecor in preventing PAF is similar to that of Amiodarone.The inhibitory action of Serecor on sino atrial node is less than that of Amiodarone.The effective dosage of Serecor is mostly below 600mg·d -1 .The side actions at the dosage up to 1200mg·d -1 increase remarkably,but the effect does not increase.
