冠状动脉旁路移植术前纵隔CT平扫对主动脉钙化诊断的临床意义

目的探讨术前常规行纵隔CT平扫对体外循环冠状动脉旁路移植术中升主动脉钙化处理方法的指导意义。方法回顾性研究2010年12月-2012年11月，于我院行体外循环下冠状动脉旁路移植术患者320例，其中男性216例，女性104例，年龄38～82岁，平均61．71岁。患者术前均常规行纵隔cT平扫检查，术中常规探查升主动脉，综合CT及术中探查结果，对于升主动脉钙化患者术中采取一次阻断，并选择非钙化部位打孔。结果纵隔cT平扫发现96例患者存在主动脉钙化，48例术中触及钙化斑块，60岁以上升主动脉钙化比例明显增高。通过上述术中处理，所有患者均顺利出院，无神经系统并发症。结论对于〉60岁的患者术前常规行纵隔cT平扫可以有效检出主动脉钙化，以指导手术方式，防止术后神经系统并发症。

Mediastinal CT plain scan in diagnosis of aorta calcification before coronary artery bypass

Objective To study the mediastinal CT plain scan in diagnosis of ascending aorta calcification before coronary artery bypass. Methods Clinical data about 320 patients （216 males, 104 females） at the age of 38-82 years （mean 61.71 years） who underwent coronary artery bypass in our hospital from December 2011 to November 2012 were retrospectively analyzed. The patients underwent routine mediastinal CT plain scan before operation. The ascending aorta was explored and blocked in patients with ascending aorta calcification and punctured at the place without calcification during operation. Results The mediastinal CT plain scanning showed aorta calcification in 96 patients. The ascending aortic plaques could be touched in 48 patients during operation, especially in those with their age 〉 60 years. The patients were discharged without complications of the nervous system. Conclusion Routine mediastinal CT scan can show aorta calcification in patients with their age 〉 60 years before operation. Complications of the nervous system should be prevented after operation.
