结核性脑膜炎患者死亡的影响因素分析

目的 探讨结核性脑膜炎患者死亡的影响因素。 方法 对西安市结核病胸部肿瘤医院2006年1月至2010年12月结核内科住院的84例结核性脑膜炎患者（A组为存活组58例，B组为死亡组26例）的临床资料（发病年龄、病程、治疗时的病期、影像学改变、是否正规治疗、有无昏迷等相关因素）进行回顾性分析，并对这些变量进行单因素Cox回归分析和多因素Cox逐步回归分析。 结果 就诊时有意识障碍或昏迷的分别为10（10/58）例和19（19/26）例，两组比较差异有统计学意义(χ²=20.760，P＜0.01)，风险比(RR值)为2.658；有脑积液者分别为38（38/58）例和18（18/26）例，两组比较差异有统计学意义(χ²=20.666，P＜0.01)，RR值为4.338；在院外接受过不正规抗结核治疗的分别为8（8/58）例和20（20/26）例，两组比较差异有统计学意义(χ²=32.196，P＜0.01)，RR值为3.551；治疗过程中出现肝损害者分别为6（6/58）例和15（15/26）例，两组比较差异有统计学意义(χ²=21.464，P＜0.01)，RR值为2.046。上述4项因素均使结核性脑膜炎患者死亡的风险增高。 结论 结核性脑膜炎患者就诊时有昏迷，治疗过程中出现肝损害、脑积液，在院外接受不正规抗结核治疗是患者死亡的危险因素。
