胃癌的综合治疗

胃癌合理治疗的前提和基础是术前准确分期,因为不同的分期其治疗方法及理念不尽相同.而胃癌的合理治疗需要多个学科共同参与.内镜治疗逐渐成为早期胃癌的首选治疗方式,但是只有多学科专家共同参与,遴选适应证,才能使早期胃癌患者真正获益于微创治疗.我国约90%的病例确诊时已属进展期胃癌,与早期胃癌相比,治疗难度更大,且预后差.只有多个学科的共同参与,才能有效地将手术、化疗、放疗、分子靶向治疗等结合为一体,制定出合理的治疗方案,使患者获益.

Multidisciplinary therapy for gastric cancer

Accurate preoperative staging is imperative in the treatment of gastric cancer since diseases in different stages mandate different management strategies. Multidisciplinary therapy is important for gastric cancer. Endoscopic treatment has become the preferred treatment for patients with early gastric cancer. However, only when multiple disciplines is involved in the decision-making process, appropriate patients can be selected and benefit from the minimally invasive treatment. In China, 90% of gastric cancers are at advanced stage at diagnosis, which are associated with worse prognosis compared to early gastric cancer. The optimal treatment depends on the multi-disciplinary therapy including surgery, chemotherapy, radiotherapy, and molecular targeted therapy.
