80例复发性气胸的临床特点和疗效分析

目的 探讨复发性气胸的临床特点及疗效,提高临床诊疗水平,减少复发.方法 回顾性分析80例复发性气胸的临床资料,并与同期80例单纯性气胸(无复发的自发性气胸)进行比较.结果 复发组中继发性气胸占65%,临床主要表现为气促、胸痛、咳嗽加重.治疗中并发皮下气肿11例,占13.75%;肺部感染4例,占5%;呼吸衰竭3例,占3.75%.选择胸腔闭式引流术的平均置管时间、平均肺复张时间较无复发组长.结论 认识复发性气胸的临床特点,早期积极治疗肺部基础疾病,选择适当的排气方法,辅助胸膜固定术可缩短疗程,减少复发.

Clinical characteristics and therapeutic effect analyse of 80 cases with recurrent pneumothorax

Objective To analyse the clinical characteristics and the therapeutic effect of recurrent pneumothorax so as to increase the level of diagnosis and treatment and decrease the recurrent rate of spontaneous pneumothorax.Methods Reviewing retrospectively 80 patients admitted with recurrent pneumothorax and comparing them with 80 patients admitted at the corresponding period with first episode of pneumothorax.Results In the group of recurrent pneumothorax,the percentage of patients with secondary spontaneous pneumothorax was 65%.The main clinical symptoms were dyspnea,chest pain,and heavier cough.The syndrome during the treatment included hypodermic emphysema(11 cases,13.75%),lung infection(4 cases,5%),respiratory failure(3 cases,3.75%).If treated with a chest tube(coupled with a water seal device),the tube stayed in the lung longer and it took more time for lung to recover in the group of recurrent pneumothorax.Conclusion To learn about the clinical characteristics of recurrent pneumothorax, to treat actively the basic lung diseases at an early stage and to choose the proper treatment(supplemented with pleurodesis),in order to shorten the period of treatment and reduce the risk of recurrence.
