Tei指数评价原发性高血压患者左心室舒张功能的价值

目的探讨心肌做功指数（Tei指数）评价原发性高血压患者左心室舒张功能的价值。方法原发性高血压患者94例分为非左室肥厚组52例，左室肥厚组42例，正常对照组30例。全部行彩色多普勒超声心动图检查测量各组左室射血分数（LVEF）、二尖瓣血流频谱舒张早期和舒张晚期血流峰值之比（E／A）和左室Tei指数。结果各组LVEF均大于〉50％，各组组间差异无统计学意义；高血压各组无论有无左室肥厚，二尖瓣E／A比值较对照组明显减小（P〈0．05）、左室Tei指数较对照组显著增大（P〈0．05）。结论高血压病早期左室结构尚未出现改变时已经有左室舒张功能的减低，左室肥厚进一步加重了左室舒张功能障碍。左心室Tei指数增加对于高血压病左室收缩功能尚未发生改变时，及早发现左室舒张功能障碍有一定意义。

The Value of Tei Index in Evaluating Left Ventricular Diastolic Function of the Patients with Essential Hypertension

Objective To discuss the value of Tel index in evaluating left ventricular diastolic function of the patients with essential hypertension. Methods 94 cases with essential hypertension were divided into 2 groups： non - LVH group （ n = 52） and LVH group （ n = 42）, another 30 normal cases formed the control group. Color Doppler examination was made to all the cases for the measttrement of the LVEF values, E/A ratio and Tel index. Results The LVEF in the 3 groups was 〉 50%, the difference was of no statistical significance; the E/A ratio of non- LVH group and LVH group decreased much more than that of the control group （ P 〈0.05） while the Tel index of left ventricle rose much more than that of the control group （ P 〈 0.05）. Conclusion LV diastolic dysfunction appear at the early stage of hypertension even when there is no structural change of the left ventricle; LV hypertrophy will enhance the LV diastolic dysfunction; the increase of LV Tei index, while the left ventricular diastolic function keeps normal, is of certain significance in diagnosing left diastolic dysfunction.
