123例艾滋病合并肺孢子菌肺炎的临床特点分析

目的:通过对123例艾滋病(AIDS)合并肺孢子菌肺炎(PCP)病例分析,探讨艾滋病合并肺孢子菌肺炎患者临床特点,提高对该病的认识,为临床诊断和治疗提供参考。方法:笔者收集河南省艾滋病临床治疗中心2008年1月至2009年12月期间收治的123例AIDS并发PCP患者临床资料进行回顾性分析。结果:123例AIDS合并PCP患者,34%已经开始接受高效抗逆转录病毒治疗(HAART),66%未接受HAART,所有患者均未正规服用复方磺胺甲噁唑片(SMZ)进行预防性治疗。CD4+T淋巴细胞计数平均值54个/mm3;并且合并多种机会性感染,主要是细菌性肺炎(85%),霉菌性口炎(75%)。结论:AIDS合并PCP病情重,死亡率高,易合并多种机会性感染,治疗难度更大,预后差。早期诊断AIDS及时接受HAART,规范服用预防用药,提高依从性,提高免疫力是关键。

The clinical characteristics of 123 cases of AIDS complicated with pneumocystis carinii pneumonia

Objective: To investigate the Clinical features of AIDS complicated with PCP,enhancing the awareness of the disease,Referencing for clinical diagnosis and treatment.Methods: We retrospectively analyzed the Clinical data of 123 cases of AIDS complicated with PCP in the clinical treatment of AIDS in Henan Province Center from January 2008 to December 2009.Results: During the 123 cases of AIDS complicated with PCP,34% were already received HAART and 66% were not.All patients had regular took SMZ for preventive treatment.The average CD+4 T lymphocyte count were 54 cells/mm3.A variety of opportunistic infections,mainly bacterial pneumonia(85%),mycotic stomatitis(75%) were combined.Conclusion: AIDS complicated with PCP is severe illness,high mortality,combinating of multiple opportunistic infections.The treatment is more difficult.The prognosis is poor.Early diagnosis of AIDS in a timely manner to accept HAART,norms taking preventive medication,improve compliance,improve immunity is the key.
