老年消化性溃疡78例分析

目的分析老年人消化性溃疡临床特征。方法将同期收治确诊的78例老年消化性溃疡病例与78例中青年消化性溃疡患者的临床情况进行对比分析。结果老年患者胃痛多不典型,而节律性空腹痛仅占21.8%,与中青年人组55.1%相比有非常显著差异(P<0.01)。十二指肠溃疡(DU)与胃溃疡(GU)发病比例,老年组0.71:l,中青年组2.75:1,两组有非常显著性差异(P<0.01)。溃疡发生部位老年组胃体部溃疡、贲门胃底溃疡、巨大溃疡均明显高于非老年组,且伴随症状多见,并发症两组无明显差异(P>0.05)。结论老年患者胃溃疡多于十二指肠球部溃疡,发生胃体部溃疡、贲门胃底溃疡、巨大溃疡较多,且症状多不典型,有节律性空腹痛者远低于非老年组患者。

The Clinical Characteristics of Peptic Ulcer in elderly patients

Objective To observe the clinical characteristics,complications and significance of peptic ulcer inelderly patients.Methods 156 patients with peptic ulcer treated during the same period were divided into twogroups,the elderly- patient group(78 cases) and the control group(78 cases),All the clinical manifestationswere evaluated.Results:In the elderly- patient group,the stomachache was often atypical and rhythmic fastingabdominal pain accounts for only 21.8 percent.As compared with 55.1 percent in control group,there wassignificant difference between two groups (P<0.01).The ration of duodenum ulcer and gastric ulcer in theelderly- patient group was 0.71.As compared with that of 2.75 in control group,there was significant differencebetween two groups (P<0 .01).The occurrences of ulcers in body,cardia and fundus of stomach and giantulcer were much higher in elderly- patient group than that in control group,The accompanying symptomswere more frequent.No significant difference of complications between two groups was found (P >0.05).Conclusions Elderly patients are more likely to have gastric ulcer which is often giant and usually locates inbody or cardia and fundus of stomach.Incidence of gastric ulcer is higher than duodenal bular ulcer in elderlypatients,The ulcers in body,cardia and fundus of stomach and giant ulcer are more freqent and thesymptoms are often atypical.The occurrence of rhythmic fasting abdominal pain in elderly- patients is much lessthan than in the control group.
