乳腺分叶状肿瘤的临床及钼靶X线分析

目的:探讨乳腺分叶状肿瘤的临床及钼靶X线表现。方法:回顾性分析5例手术病理证实的乳腺分叶状肿瘤的临床及钼靶X线资料。结果:5例均为女性,年龄30~50岁,平均年龄(40.3±7.76)岁,以发现乳腺无痛性包块就诊,病程1个月至10年,3例有近期迅速增大。触诊肿块质地坚韧,表面光滑,无压痛,边界清楚,活动度好。钼靶X线片显示瘤体4个,多位于外上象限(3/5),直径2.0~4.5 cm,平均(3.6±0.75)cm,为高密度或等密度肿块,分叶状3个,椭圆形1个,1例只显示右乳非对称性致密影,未见肿块影。4例瘤体显示部分边界光滑锐利,其中3例可见线样透亮环。结论:对于年龄较大、短期内迅速增大的乳腺无痛性包块,钼靶X线片呈"良性肿瘤"表现,特别是显示分叶状边缘的,应考虑分叶状肿瘤的可能。

Clinical and mammographic characteristics of phyllodes tumor

Objective:To investigate the characteristics of clinical and mammographic findings of breast phyllodes tumors.Methods: The data of 5 involved cases confirmed both postoperatively and pathologically were analyzed retrospectively.Results: The five patients were all females aged between 30 and 50 with an average of 40.3 years old.Each patient visited the doctor because of unexpected detection of indolent mass in breast and the symptom progressed from 1 month to 10 years.Rapid growth of the mass was seen currently in 3 cases.On the physical examination,the lesions were firm but mobile,with smooth surface,no tenderness and sharply defined outline.The mammographic findings suggested that the four masses were sit mostly on the upper outer quadrant in anatomic structure(66.7%), sized in diameter between 2.0 and 4.5 cm with an average of 3.6 cm.All lumps were high density or equal density,shaped lobulately(n=3) or ovally(n=1). Only one case demonstrated local high density but with no shadow zone.The mass in four cases were well-defined,in which three showed a clear partial halo.Conclusion: In older female cases,provided that the mass in breast grow rapidly in short period,even though the mass is painless with mammographic findings of seemingly benign,it may be considered as phyllodes tumor,especially that of lobulately shaped.
