瘦素水平在原发性肾病综合征激素治疗中的意义

目的：观察原发性肾病综合征（primary nephrotic syndrome，PNS）患儿糖皮质激素治疗前后血清瘦素水平的变化。方法：采用ELISA方法检测PNS患儿激素治疗前及规范激素治疗8—10周后的血清瘦素水平，并与年龄、性别、体重指数（body mass index，BMI）均匹配的28例健康儿童进行比较。全自动生化分析仪测定患儿血清总胆固醇（CHOL）、甘油三酯（TG）水平，计算BMI。结果：PNS组激素治疗前瘦素水平与治疗后比较，差异具有显著性（P〈0．01）；PNS组激素治疗前与正常对照组瘦素水平比较，差异无显著性（P〉0．05），PNS组激素治疗后瘦素水平与正常对照组比较有明显升高，差异具有显著性（P〈0．01）。PNS组激素治疗前后与正常对照组BMI比较差异无显著性（P〉0．05）。激素治疗前与激素治疗8～10周后TG、CHOL差异无显著性（P〉0．05）。结论：糖皮质激素治疗PNS可致血清瘦素水平升高。

Clinical Significance of Serum Leptin Levels in Glucocorticosteroid Treatment for Children with Primary Nephritic Syndrome

Objective： To observe the serum leptin level change in children with primary nephritic syndrome （PNS） before and after the treatment of glucocorticosteroid. Methods： Serum leptin levels of 26 PNS children （before and after 8 -10-week standard hormone treatment） and 28 healthy children （control group） with matchable age, sex and body mass index （BMI） to them were determined with ELISA. The serum total cholesterol （CHOL） and triglyeride （TG） were determined with automatic biochemical analyzer and BMIs were calculated. Results： The serum leptin level of PNS patients before treatment did not differ from that of control group （ P 〉 0.05 ）, but differed from that after the treatment （P 〈 0.01 ）. The latter one was significantly higher than that of control group （P 〈 0.01 ）. No significant difference of BMI was found between PNS children （both before and after treatment） and control group （ P 〉 0.05 ）. There was no significant difference of TG, Or CHOL in PNS children before and after treatment. Conclusion： Glucocorticosteroid treatment could raise serum leptin level in PNS patients.
