经阑尾残端逆行置管减压治疗粘连性肠梗阻

目的：探讨经阑尾残端逆行置管减压治疗粘连性肠梗阻的临床效果。方法：收集我院1999～2007年住院治疗的粘连性肠梗阻78例，对其进行回顾性分析，并根据治疗方式分为置管组和对照组，比较置管减压治疗粘连性肠梗阻的临床效果。结果：置管组术后肛门排气时间为（22．9±9．4）h，明显低于对照组术后肛门排气时间（57．3±15．5）h，P〈0．01；置管组住院时间为（12．0±2．2）d，明显低于对照组住院时间（18．2±4．5）d，P〈0．01；置管组切口感染或裂开率为5．0％，明显低于对照组切口感染或裂开率23．7％，P〈0．05；置管组再次梗阻率为0，明显低于对照组再次梗阻率23．7％．P〈0．05。结论：经阑尾残端逆行置管减压治疗粘连性肠梗阻优于常规手术治疗。

The clinical study of decompression in the treatment of adhesive ileus with intubation from appendiceal stump

Objective：To investigate the clinical effect of decompression in the treatment of adhesive ileus with intubation from appendical stump retrograde. Methods ：78 cases of adhesive ileus from 1999 to 2007 were studied retrospectively. All cases were divided into intubation group and control group according to different therapy, the clinical effect were compared. Results：The intubation group＇s time of passage of gas by anus was （22.9±9.4） h, which was lower than control group＇s （57.3±15.5） h,P〈0.01 ;the length of stay of intubation group was （12.0±2.2） d,which was lower than control group＇s （18.2± 4.5） d ,P〈0.01 ;the infection rate or split rate of incisional wound of intubation was 5.0%,which was lower than control group＇s 23.7% ,P〈0.05 ;the obstruction rate of intubation group was 0,which was lower than control group＇s 23.7% ,P〈0.05. Conclusion： Decompression treatment in adhesive ileus intubation from appendiceal stump is better than common operation.
