咳嗽变异性哮喘12例误诊分析

目的探讨咳嗽变异性哮喘(CVA)的误诊的原因及诊断方法。方法12例CVA患者采用支气管舒张实验,辅助X线和心电图检查,并结合临床表现进行诊断。结果X线胸片检查正常9例,双肺纹理增粗2例,右下肺纹理增粗1例;心电图检查正常7例,ST-T段改变2例,偶发室性期前收缩1例,窦性心律不整2例;支气管舒张实验阳性7例。12例均有反复发作的阵发性咳嗽,无喘息症状,且未闻及哮鸣音。结论CVA患者只要有反复发作阵发性咳嗽,胸片正常者,并支气管扩张实验阳性者应考虑本病并做相应鉴别诊断。

Analysis of misdiagnosis of 12 cases of cough variant asthma

Objective To explore causes of misdiagnosis of cough variant asthma(CVA). Methods Twelve CVA patiens were diagnosed by bronchodilatation test in addition to diagnosis by the method of chest X-ray,electrocardiography and combination of clinical manifestation.Results Among 12 patients,9 cases were normal in chest X-ray,lung markings enhancement was in 2 cases of sides of lung and right subpulmonary markings enhancement was in 1 case,7 cases were normal in electrocardiography,changes of ST-T segment were in 2 cases,1 case was ventricular premature beat,2 cases were sinus arrhythmia,7 cases were positive in bronchodilatation test.Recurrent attacks cough occurred in 12 cases and None manifestation of gasping occurred and no wheezing rale was in the lungs.Conclusion Patients with could be diagnosed as CVA when there are manifestation including recurrent attacks paroxysmal cough,normal sternite and positive bronchodilatation test and should be made a differential diagnose.
