18F-FDG PET/CT显像在老年肺癌诊疗中的价值

目的 探讨^18F-氟代脱氧葡萄糖（^18F—FDG）PET／CT显像对老年肺癌诊断和治疗的临床应用价值。方法 48例老年肺癌患者，在PET／CT显像前、后，根据国际抗癌联盟（Intemadonalnion Against Cancer，UICC）1997年肺癌TNM分期标准进行分期，并根据美国国家综合癌症网络（National Comprehensive Cancer Network，NCCN）公布的2005年肺癌临床指引，拟定治疗策略，比较检查前、后变化。结果37例患者肺部病灶^18F—FDG摄取增高，标准化摄取值（SLIV）〉2．5，在PEF／CT显像后，12例分期调高，分期调高率25．0％（12／48），11例发现远处转移，手术切除概率减少了23．9％。40例患者接受了PET／CT机定位及PET／CT融合图像为依据肿瘤靶区勾画的三维适形放疗。结论 ^18F—FDGPET／CT显像对老年肺癌的诊断、分期、治疗策略的制定及肿瘤靶区的勾画等方面具有较大的价值。

Value of 18F-FDG PET/CT Imaging in Diagnosis and Treatment of Lung Cancer in Elderly

Objective To explore the clinic value of ~(18)F-FDG PET/CT imaging in the diagnosis and treatment of lung cancer in elderly.Methods ~(18)F-FDG PET/CT was performed in 48 elderly patients with lung cancer.Cancer staging was determined by using TNM and UICC staging systems in 1997.The planning of cancer treatment was made by using NCCN Guidelines for lung cancer in 2005.The differences of cancer staging and treatment planning before PET/CT were compared with those after PET/CT.Results The lesions of lung cancer in 47 patients showed intense uptake of ~(18)F-FDG with the value of SUV more than 2.5.After PET/CL the rate of increased staging was 25%(1 2/48).The operation possibility was decreased 23.9%because of 14 distant metastases were detected in 11 patients.Forty patients had received 3-dimensional conformal radiation therapy localized by using PET/CT machine.All of grass target volumn.Radiation planning were decided according to positive PET/CT fusion images.Conclusion ~(18)F-FDG PET/CT fusion imaging is a valuable technology in the staging,diagnosis,decision of cancer treatment planning and grass target volumn of lung cancer in elderly patients.
