纤维支气管镜及肺功能检查在气管肿瘤中的应用

目的探讨无痛性纤维支气管镜及肺功能检查在原发性气管肿瘤中的应用。方法对气管原发性肿瘤30例患者行无痛性纤维支气管镜及肺功能检查并进行回顾性分析。结果30例均行纤维支气管镜检查和肺功能检查。纤维支气管镜检查示瘸体位于气管上段5例，中段16例，下段9例；气管1／3阻塞者17例，气管2／3阻塞者11例，气管几乎完全阻塞者2例；按流速-容量曲线图形对气管肿瘤分型并分为3组：胸外可变型12例为A组，胸内可变型10例为B组，固定性狭窄型8例为c组。B组、c组呼气峰流速（PEF）、1秒量（FEV1）、1秒率（FEV1／FVC）、用力呼气早期流速（FEF25）、平均呼气中期流速（FEF25-75）较A组显著下降（P〈0．05）；A组、c组吸气峰流速（PIF）显著低于B组（P〈0．05）。结论纤维支气管镜结合肺功能检查评价气管肿瘤患者通气功能有一定临床价值。

Fiberoptic bronchoscopy and pulmonary function tests for evaluation of ventilation function in patient with primary tracheal tumors

Objective To observe the value of pain free fiberoptic bronchoscopy and lung function tests for evaluation of ventilation function in primary tracheal tumor. Methods Thirty cases of primary tracheal tumor who received pain free fiberoptic bronchoscopy and lung function tests were enrolled in this study. Results Fiberoptic bronchoscopy showed upper tracheal tumor in 5 cases, middle tracheal tumor in 16 cases, lower tracheal tumor in 9 cases. 1/3 tracheal obstruction occurred in 17 cases, 2/3 obstruction in 11 cases, nearly 100% obstruction in 2 cases. Based on velocity - capacity curve, patients were divided into 3 groups. PEF, FEV1, FEV1/FVC, FEF 25, FEF 25-75 of group B and group C were significantly lower than group A. PIF of roup A and group C were significantly lower than group B, Conclusion Combined Fiberoptic bronchoscopy and pulmonary function tests show good value for evaluation of ventilation function in patients with tracheal tumors.
