必可酮长期吸入治疗小儿哮喘对生长发育、肺功能及垂体肾上腺轴影响的研究

摘    要:目的 探讨必可酮 (二丙酸倍氯米松 ,BDP)长期吸入治疗小儿哮喘对生长发育、肺功能及垂体肾上腺轴的影响。方法 选择 80例哮喘儿童于给药前、给药后 4周、3、6、12、2 4、36个月测定最大呼气峰流速值 (PEF)、ACTH、血、尿皮质醇 ,治疗前、治疗后 1、2、3年测量身高、体重 ,并与正常健康体检儿童 80例作对照。结果 长期吸入BDP治疗有效率为 90 0 % ,哮喘患儿于BDP治疗前、治疗后 4周、3、6、12、2 4、36个月ACTH、血、尿皮质醇水平与正常健康儿童相比差异无显著性 (P >0 0 5 ) ,PEF水平在治疗后 6个月与正常健康儿童相接近 (P >0 0 5 )。结论 长期吸入BDP是治疗小儿哮喘的理想药物 ,不仅能及早改善肺功能 ,而且在标准推荐剂量范围内使用对生长发育、垂体肾上腺轴无明显影响。

Abstract:Objective To inquire into the effects of inhaling becotide(Beclomethasone Dipropionate,BDP) inhaler for a long time for the treatment of asthma in children on the growth development,lung function and glandula-adrenal glands stalk of the children.Method Maximun expiratory flow rate(PEF),ACTH,blood and urine hydrocortisone were tested in 4th week,3rd,6th,12th,24th and 36th month before and after given the medicine from 80 selected children asthma sufferers.Body weight and body height were examined before the treatment and 1st,2nd and 3rd year after treatment and comparison was made with 80 cases of normal healthy children.Result The effective rate of inhaling BDP for a long time for the treatment of asthma in children was 90.0%.Compared with the normal healthy children,no difference could be seen of the ACTH,blood and urine hydrocortisone in children who suffered from asthma before the treatment and 3rd,6th,12th,24th and 36th month after treatment by BDP.PET level was almost as the normal healthy children 6 months after treatment (P>0.05).Compared with the normal healthy ones,no difference was observed of the body height and body weight in 1st?2nd and 3rd year after treatment (P>0.05).Conclusion It is an ideal method to treat the asthma in children by inhaling BDP for a long time.It could not only improve the lung function early but also results no obvious negative effects on the growth development and glandula-adrenal glands stalk of the children within the recommended standard dose.
