原发性十二指肠恶性肿瘤17例报告

1972～1987年收治原发性十二指肠恶性肿瘤17例。上腹痛,呕吐、出血和黄疸为主要症状,症状常与肿瘤所在部位有关。乳头周围的6例全部作了胰十二指肠切除术,而乳头上方的4例仅能行姑息性手术。乳头下方的7例中,只有3例可行胰十二指肠切除。9例胰十二指肠切除病例中,3例存活5年以上。由于本病缺乏特异性症状,对可疑病例及早作低张十二指肠造影和纤维十二指肠镜检查有助于早期诊断。

Primary Malignant Neoplasms of the Duodenum.A Report of 17 Cases

Seventeen patients with primary malignant neoplasms of the duodenum were treated from 1972 to 1987. Epigastric pain, vomiting, jaundice and hemorrhage were the main symptoms, which usually related to the sites of the tumors. Pancreaticoduode-nectomy was performed on all the 6 cases with periampullary tumors while only palliative operations were performed on all the 4 cases with supraampullary tumors. Among the 7 cases with infraampullary tumors, only 3 patients could be treated with pancreaticoduodenectomy. Of 9 cases treated by pancreaticoduodenectomy, three survived for more than 5 years. In view of the fact that the disease has no characteristic symptom, it is mandatory to perform hypotonic duodenography and fiberoduodenoscopic examination on individuals of suspicion as early as possible.
