CUL4B表达与结直肠癌临床病理特点及预后的关系

目的：探讨CUL4B在结直肠癌组织中表达及其与患者临床病理参数和预后的关系。方法选取2010年1月至2010年12月间在本科收治的102例结肠癌患者临床资料，应用免疫组化法检测102例结直肠癌组织中CUL4B蛋白表达情况，分析CUL4B蛋白表达与结直肠癌临床病理参数及预后的关系。结果 CUL4B蛋白在结直肠癌组织中阳性表达率为55．88％（57／102），CUL4B蛋白阳性表达与淋巴结转移（P＝0．010）、肿瘤远处转移（P＝0．009）、pTNM分期（P＝0．001）、Duke's分期（P＝0．002）、肝转移（P＝0．015）和腹膜转移（P＝0．025）有明显相关性。CUL4B在结直肠癌组织中的阳性表达提示患者预后较差（P＜0．001）。 COX多因素回归分析结果表明淋巴结转移（P＜0．001）、血管侵犯（P＝0．038）和CUL4B高表达（P＜0．001）是判断结直肠癌患者预后的独立危险因素。结论 CUL4B蛋白阳性表达可作为结直肠癌患者预后不佳的评价指标之一。

CUL4B expression and its relationship with clinicopathological features and prognosis in colorectal cancer

Objective To investigate the expression of CUL4B protein and its relationships with clinicopathological parameters and prognosis in colorectal cancer. Methods Clinicopathological data of 102 patients with colorectal cancer admitted to our department from January 2010 to December 2010 were analyzed retrospectively..The expression of CUL4B protein in tumor tissues was assessed by immunohistochemistry method..The relationships between CUL4B expression and clinicopathological parameters as well as prognosis were analyzed. Results Positive rate of CUL4B expression in primary colorectal cancers was 55.88%(57/102). CUL4B expression was significantly correlated with lymph node metastasis (P=0.01), distance metastasis (P=0.009), pTNM staging (P=0.001), Duke's staging (P=0.002), liver metastasis (P=0.015) and peritoneal metastasis (P=0.025). Positive expression of CUL4B indicated a worse prognosis in patients with colorectal cancer (P<0.001). Multivariate COX regression analysis revealed that lymph node metastasis (P<0.001), vascular invasion (P=0.038) and CUL4B positive expression (P<0.001)were independent prognostic factors for colorectal cancer patients. Conclusions The positive expression of CUL4B was an independent prognostic factor for patients with colorectal cancer.
