经自制硬直镜行肺肿瘤切除术--附22例报告

目的 :介绍一种新的微创技术———经自制硬直镜行肺肿瘤切除术。方法 :总结分析 1993年 3月～ 1999年 3月采用本法完成的肺楔型切除术 18例 ,左肺下叶切除术 1例 ,右肺中下叶切除术 1例 ,右肺下叶背段切除术 1例 ,纵隔肿瘤切除术 1例的临床资料。结果 :手术切口长 4～ 8cm ,手术时间 15～30min ,术中出血 2 0～ 50ml ,均未输血 ,无术后并发症 ,术后平均住院时间 8.5天 (不包括肺叶切除者 )。结论 :与标准剖胸手术相比 ,本术式具有创伤小 ,手术时间短 ,术中出血少 ,无需输血 ,无术后并发症 ,术后切口疼痛轻 ,下床活动早 ,术后住院时间短等优点 ,可作为肺良性肿瘤手术治疗的常规术式 ,也可应用于肺恶性肿瘤减瘤术

Self-made rigid metal endoscope used for the surgical treatment of the lung tumor:a report of 22 cases

Objective:To introduce a new minimally invasive technique:selfmade rigid metal endoscope used for the surgical treatment of lung tumor.Methods:22 cases with lung tumor underwent surgical treatment by the selfmade rigid metal endoscope,from march 1993 to march 1999 consisted of 6 pulmonary hamartoma,6 pulmonary inflammatory pseudotumor,4 pulmonary tuberculosis,3 pulmonary squamous cell carcinoma,1 pulmonary carcinoid,1 pulmonary metastatic tumor and 1 mediastinal tumor.Among them,18 patients underwent wedge resection,1 segment resecion,2 lobectomy, l doublelobectomy and l resection of mediastinal tumor. Results:The length of incision ranged from 4 to 8 cm,the operating time from 15 minutes to 30 minutes,the amount of the bleeding during the operation from 20 ml to 50 ml and no need of blood transfusion. No need of the drainage of pleural cavity except for lobectomy.No postoperative complications occurred.On the first day after operation the patients could get off the bed and move about. The average postoperative hospital stay was 8.5 days except for lobectomy.Conclusions:Compared with the standard thoractomy,the advantage of the thoractomy by selfmade rigid metal endoscope includes:minimally invasion,shorter operative time,minimally bleeding,no need of blood transfusion,no postoperative complications,less suffering,getting off the bed early and shorter time in postoperative hosptalization.The procedure may be used as the common surgical treatment of pulmonary benign tumors and the palliative surgical treatment for malignant tumors of the lung and more suitable for the aged and those can not endure the standard thoractomy for the deficiency of the cardiopulmonary function.
